I
hereby acknowledge receipt of the animal described on this Adoption form in
return for which I agree to be bound by and to comply with, all of the following
terms and conditions of adoption.
1.
I will
provide proper and sufficient food, water, shelter, and kind treatment for the
animal at all times.
2.
I will
have the animal immunized, kept on heartworm preventative (Interceptor only) and examined annually at my expense.
3.
I will
provide an I.D. tag with my name, and phone number and keep it on the Collie at
all times.
4.
I will
not sell or abandon this animal, and agree that if I no longer desire to retain
the animal, I will return it to the COLLIE CLUB OF AUSTIN RESCUE.
5.
I will
not allow the animal to be used for the purposes of experimentation.
6.
I will
agree to immediately retrieve the animal at my expense from any public pound or
animal shelter when notified that the animal is being held at any such location.
In the event the animal gets lost, I will make every reasonable effort
and attempt to relocate and claim the animal.
7.
I agree
that I will at no time assert any claim, charge, or demand of any kind against
COLLIE CLUB OF AUSTIN RESCUE for any expenses, which may have been incurred by
me, including Veterinary fees, in connection with this animal.
I also agree that any illness or injury occurring after adoption will be
my sole responsibility.
8.
I hereby
accept the animal as is, assume all risk of ownership of the animal, including
the risk of injury or damage caused by the animal.
I ACKNOWLEDGE THAT I HAVE READ AND FULLY UNDERSTAND THE TERMS AND CONDITIONS OF THIS ADOPTION AGREEMENT AND THAT I WILL COMPLY WITH ALL THE TERMS ABOVE.
